What Is Spravato (Esketamine) Treatment?
Spravato is an FDA-approved prescription nasal spray containing esketamine. It is approved for adults with treatment-resistant depression and for depressive symptoms in adults with major depressive disorder who have acute suicidal thoughts or actions, and it is always taken alongside an oral antidepressant. Unlike SSRIs, which act on serotonin and can take four to six weeks to work, esketamine acts on the brain’s glutamate system, which is why some patients notice a change within the first one to two weeks.
Because of its effects, Spravato cannot be picked up at a pharmacy. It is only administered in a certified treatment setting with a health-care professional present. Your Spravato Session at Our Olathe Office
Treatment starts with a psychiatric evaluation to confirm the diagnosis, review the antidepressants you have already tried, and check for conditions that rule Spravato out. If it is a fit, our office helps with the insurance benefits check and prior authorization before your first dose.
- Induction phase: two sessions a week for the first four weeks.
- Maintenance phase: weekly sessions in weeks five through eight, then weekly or every other week based on how you respond.
- Each visit: you self-administer the spray under supervision, then rest in a private room for the required two-hour observation while our team monitors your blood pressure and how you feel. Bring a book or headphones.
- Getting home: you cannot drive until the next day after a full night’s sleep, so plan a ride. Skip food for two hours and liquids for 30 minutes before your appointment to reduce nausea.

Who Is a Candidate for Spravato?
Spravato is for adults 18 and older with major depressive disorder who have not had adequate relief from at least two different oral antidepressants taken at an adequate dose and duration. It is also used alongside an oral antidepressant for adults whose depression includes suicidal thoughts.
It is not a fit for everyone. Our providers screen for aneurysmal vascular disease, arteriovenous malformation, a history of intracerebral hemorrhage, uncontrolled high blood pressure, pregnancy and breastfeeding, and allergy to ketamine or esketamine. A history of substance misuse does not automatically rule you out, but it is part of the conversation. The only way to know is a consultation with one of our psychiatric providers.
Potential Benefits of Spravato
In the clinical trials that led to approval, adults with treatment-resistant depression who added Spravato to a new oral antidepressant were significantly more likely to reach remission than those who added placebo, and patients who stayed on Spravato were about half as likely to relapse. Patients often describe the first change as a loosening of the heaviness and hopelessness, which makes the work in therapy possible again.
